The PhilHealth online registration is easy and straightforward. Here is the step-by-step guide that include photos:

STEP 1: Go to the PhilHealth Electronic Registration System. Click “PROCEED”


STEP 2: Read the Terms and Conditions carefully,  tick the checkbox saying that you agree to the conditions and then click Accept.


STEP 3: On the PhilHealth online registration form, fill out the PhilHealth online application form with the following sections:
  • Personal Information
  • Contact Details
  • Address
  • Dependents Information
  • Guardian Information
  • Membership Category Information
  • Upload Documents (optional)

Note: if you’re single, list your parents who are aged 60 years or older as your dependents. If you’re married, list your spouse and your children as your dependents.

STEP 4: Upload any supporting documents such as a birth certificate, passport or driver’s license. To upload your documents, scan them and save them as JPEG, PNG, GIF or PDF files, and then upload them one by one. If you have dependents, upload their documents as well.


STEP 5: Review all the information that you entered. Enter the provided Captcha code, tick the small box below, and click “Submit Registration.”


STEP 6: You’ll receive instructions via email on how to complete your PhilHealth membership application. Copy the transaction number, which serves as a reference to your registration.

It is important to remember to record your PhilHealth ID number (PIN) once it’s issued, since it’s permanent and you cannot get a new one. You need to start paying your premiums to become an active member and become eligible for benefits.
